What technology is 'Real-time Database' built on? Is it a database or a Cache? If it is a Cache, is this an application cache?

Probably a bit confusing terminolgy used from my side, but the real-time database is RAM memory of the SCADA system, not a SQL like database. Best to compare this as application cache memory.
So, when the SCADA system is switched off, the real-time data is lost.
The Real-Time data consist or variables/TAGs filled by data coming from the connected PLCs/RTUs.
These TAGs can be used to visualize, detct alarm levels and LOG to a historical database. The historical database (historian) is a file or database on a harddisk/fileserver.
